Important Forms

Below are forms used during the course of the school year. Click on the appropriate form, print on your local printer, complete, and return according to the instructions included on the form.

All band students are required to submit a Contact Information Form. If your contact information has changed you can submit a contact update.

Due to a county wide freeze on activity fees the band program mostly relies on your donations and commitment to continue operating at a superior achievement level. Please consider making a tax deductable patron donation this year.

When chaperoning any trip please review and complete a Chaperone Form. This form is required by the county and describes your duties.

In order to chaperone any band event, all chaperones must complete in advance the Child Abuse Awareness Training and have the completion certification on file. This year Howard County has made this 5 minute training available online. To complete the training watch the on-line video, print and complete the Certificate of Completion. Please have your child place your certificate in the proper folder in the band room and we will forward your information to Ms. Julie Prince.

For extended day or overnight trips all parents must complete an Emergency Procedure/Health Information form.

A Medication/Treatment Form must be completed by your health care provided for each trip if your child requires any medication. It must be completed and signed by a physician.
